PRIVACY POLICY

John Curtin Aged Care Inc is committed to maintaining the privacy of personal and health information. We are bound by the Australian Privacy Principles and Victorian Health Privacy Principles and have embraced these principles as part of our standard operating procedures.

Restricted access to, and secure storage and disposal of information and records identified as confidential (both hard and soft copy) will be maintained.

An aged care facility must collect personal information as part of the application and assessment process for accommodation. This information may be of a personal nature and can include medical and social history, financial information and other personal history that may be of a sensitive and private nature. The information will allow John Curtin Aged Care to assess the urgency of your application and your suitability to fill a particular vacancy.

 

In order to provide appropriate placement, care and support, John Curtin Aged Care may collect personal and health information from individuals and families verbally and in writing, including contact and next of kin information, date of birth, health information and financial information. This information is also stored electronically. This information will be used to ensure individuals are eligible and suitable for our services. To enable us to meet our care obligations, and as required by legislation, information provided may need to be disclosed to facility personnel (i.e. nurses, staff, accountants) and consulting health professionals (i.e. medical practitioners, physiotherapists, pharmacists, pathologists, dietitians, social workers, psychologists, occupational therapists) and other service providers.

If you are concerned about the handling of your health or personal information, or would like to know more about our privacy practices, including your right to request access to your information, please contact our Privacy Officer, the CEO.

All complaints are taken seriously and we will endeavour to deal with them promptly.

 Requests for access must be in writing. Our Privacy Officer will arrange for an access form to be sent to you and can assist with any enquiries you may have about the process. We will respond to requests for access within a reasonable period after the request is made, usually within 14 days.

We may also conduct surveys and gather additional information for quality improvement, this information is aggregated and non-identified.  We have no overseas business and therefore no personal information will be disclosed to any overseas recipients.
